Position Summary
We’re hiring a Connecticut-licensed Advanced Practice Provider (APP)—either a Nurse Practitioner or Physician Assistant—to deliver high-quality telehealth obesity care. You’ll conduct virtual visits, collaborate with an interdisciplinary team, and receive mentorship in obesity medicine from co-founder & CMO Dr. Jamie Kane. Flexible part-time hours can expand to full-time as patient volume grows.
Key Responsibilities
- Telehealth Visits: Conduct real-time video visits and follow-up consultations for a variety of healthcare services, including chronic care management, remote patient monitoring, and urgent care needs.
- Care Coordination: Collaborate with our care team and our partner’s care teams (physicians, nurses, dietitians, behavioral health specialists, and health coaches) to develop comprehensive and individualized care plans.
- Patient Education & Support: Provide health education and counseling to patients and caregivers, helping them navigate treatment options and manage their conditions effectively.
- Clinical Documentation: Maintain accurate, up-to-date electronic health records (EHR), including patient histories, treatment plans, and follow-up recommendations.
- Prescription Management: Evaluate patient needs and prescribe appropriate medications as needed, adhering to state regulations and clinical guidelines.
- Quality & Compliance: Follow clinical protocols, best practices, and relevant regulatory standards to ensure the highest quality of care.
- Technology Utilization: Leverage telehealth platforms, communication tools, AI based platforms, and EHR systems to deliver efficient and effective virtual care.
Requirements
Qualifications
- Licensure: Active and unrestricted Physician Assistant or Nurse Practitioner license in the State of Connecticut (required).
- Education: Master’s or Doctoral degree from an accredited Nurse Practitioner program (required).
- Certification: Board-certified as a Nurse Practitioner (e.g., ANCC or AANP) in a relevant specialty (Family, Adult, Geriatric, etc.) (strongly preferred). NCCPA-certified PA-C.,
- Experience: Minimum of 1 year of clinical experience practicing independently as a Nurse Practitioner (required).
Telehealth or virtual care experience (preferred). - Experience in obesity medicine, nutrition and lifestyle modification (preferred)
- Medicare and Medicaid care delivery experience (preferred)
- Technical Skills: Comfortable using telehealth platforms, EHRs, Microsoft Teams workspace
- Other Requirements:
